Patient and Nurse
  • Assesses and evaluates the health status of resident / patient and provides care and treatment in accordance with physician orders and standards of practice

  • Assesses patients by physical examination including pertinent diagnostic testing to determine health status

  • Administers medications and treatments

  • Participates in the care planning process and oversees implementation of the plan

  • Supervises LPNs and nursing assistants

  • Communicates with physicians regarding changes in conditions, diagnostic test results, etc

  • Documents assessments and care in compliance with standards of care and company policy

  • Educates patients and their families on health-related issues

  • Other duties may be assigned

  • Employee will occasionally assist to lift and / or transfer patients

Preparation for Blood Test
  • The LPN/LVN works under the direction of physicians and registered nurses (RNs)

  • Assesses patients by physical examination, including pertinent diagnostic testing to determine health status

  • Administers medications and treatments

  • Participates in the care planning process

  • Supervises LPNs and certified nursing assistants

  • Communicates with physicians regarding changes in resident's conditions, diagnostic test results, etc

  • Documents assessments and care in compliance with standards of care and company policy

  • Educates patients and their families on health-related issues

  • Completes required forms and documents in accordance with company policy and state and/or federal regulations

  • Employee is frequently required to stand, walk, use hands or fingers, reach with hands and arms, feel, talk and hear

  • Employee is occasionally exposed to blood or other body fluids, fumes or airborne particles and toxic or caustic chemicals

Medical Worker
  • Assisting residents with activities of daily living such as transferring, turning and positioning, bathing, , dressing, grooming, feeding, toileting and ensuring residents are clean.

  • Upon request by the Director of Nursing or licensed nurse unit manager, obtaining vital signs and weights and monitoring food and liquid intake.

  • Responding to resident and family needs respectfully and with a positive manner in all interactions.

  • Making routine care rounds on assigned residents in accordance within established guidelines.

  • Reporting data in resident records consistently, accurately and in a timely manner.

  • Promptly reporting suspected or actual resident changes of condition to the licensed nurse.

  • Complying with procedures for infection prevention including, but not limited to, hand washing, transmission-based precautions, TB and Hepatitis B requirements and blood borne pathogen regulations.

  • Following safety rules and requirements including lift and transfer procedures, hazard communication, electrical safety, slips, trips and falls policies, use of personal protective equipment and elopement procedures.

Taking Care of Plants

The Home Care Supervisor is accountable for ensuring the overall quality and appropriateness of patient care and nursing services for all home care clients as
 

  • Refer patients needing other resources of care to appropriate services

  • Consult with family, physicians, institutional staff, clients, nurses, and other disciplines while establishing a coordinated home care plan

  • Intake data, essential background information, care plan, and signed physician orders.

  • Oversee the initial and ongoing assessment of home care clients

  • Complete a nursing assessment of all home care clients within 48 hours of order

  • Initiate, review, and revise the plan of care

  • Obtain a physicians plan of treatment/care and ensure their execution

  • Review timesheets bi-weekly and ensure that tasks match the care plan

  • Assess the patients overall health status and home safety environment

  • Review and determine that care is provided according to the care plan

  • Determine care is appropriate to identified needs of the patient

  • Revise the care plan as needed

  • Determine care is being provided on assigned days and time as scheduled

  • Determine that staff member is oriented to assigned responsibilities and the

  • Review bloodborne pathogens and exposure control with each employee

  • Handle and report problems to appropriate parties

Home Nurse Making Bed

  • Caregivers must comply with state background screening requirements

  • Responsibilities

  • As a Personal Care Attendant, you'll work 1-on-1 with your clients inside their homes, and support them with the following types of activities:

  • Light housekeeping, including organizing, laundry and basic cleaning Personal activities such as dressing, medication and assisting with meals Accompanying your clients to and assisting them with any activities outside the home such as grocery shopping or running errands
