As part of our onboarding process, we will be conducting a pre-employment background investigation in junction with your application for employment with our company. Our onboarding process may involve verifying or reviewing any of the following relevant information: Criminal Convictions, Prior Employment History, Educational History and Office Urine Drug Screen.

PRE-EMPLOYEE AGREEMENT

Part of our onboarding/hiring process includes testing for controlled or illegal substances and the completion of a background check. If you wish to complete the onboarding process, you must complete all of the onboarding requirements which include consent to such testing/screening by signing this form.

I also hereby authorize Competent Medical Solutions and/or its agents to make investigation of my background, references, past employment, education, and criminal history. I consent freely and voluntarily to the requests.
